Product Details

PROLOMET-XL 12.5 Tablet is extended release formulation used to reduce high blood pressure and relieve chest pain caused due to reduced blood flow to the heart. It is also used for treating other complications associated with the heart and blood vessels.

 

Indications:

  • Hypertension
  • Angina pectoris
  • Heart Attack
  • Congestive Heart Failure

Dosage:

  • Hypertension: 25-100 mg PO qDay initially; may be increased at intervals of 1 week or longer; usual range, 50-100 mg/day; not to exceed 400 mg/day.
  • Acute Myocardial Infarction: 25 mg PO qDay initially; increased every 2 weeks PRN; target dosage, 200 mg/day.
  • Angina: 100 mg/day PO initially; may be increased at intervals of 1 week or longer; not to exceed 400 mg/day.
  • Acute Tachyarrhythmia (Off-label): 5 mg IV over 1-2 minutes q5min; total dose not to exceed 15 mg.
  • Atrial Fibrillation/Flutter or Supraventricular Tachycardia (Off-label): 2.5-5 mg IV q2-5min; not to exceed 15 mg over 10-15 minutes; maintenance: 25-100 mg PO q12hr.

Onset of Action: The effect of this medicine can be observed within 1 hour of administration of the dose.

Duration of Action: The effect of this medicine lasts for an average duration of 12 hours.

 

Side effects:

  • Chest pain and discomfort
  • Decreased blood pressure
  • Blurred vision
  • Dizziness
  • Slow heart beat
  • Shortness of breath
  • Nausea or Vomiting
  • Skin rash

Interactions:

  1. Interaction with Medicines: Severe to moderate interaction with Ergotamine, Amlodipine, Aminophylline, Diltiazem.

  2. Interaction with Diseases: 
    Asthma: This medicine is not recommended for use in patients with bronchial asthma or chronic obstructive pulmonary disease since it may worsen the patient's condition. Replacement with a suitable alternative should be done under your doctor's supervision.
    Diabetes: This medicine should be used with caution in patients with diabetes mellitus due to the increased risk of altered blood glucose levels. Close monitoring of blood glucose levels is advised for such patients. Replacement with a suitable alternative may be required based on the clinical condition.
    Liver Disease: This medicine should be used with caution in patients suffering from liver diseases due to the increased risk of severe adverse effects. Appropriate dose adjustments or replacement with a suitable alternative may be required based on the clinical condition.
    Glaucoma: This medicine should be administered with caution in patients suffering from glaucoma due to the increased risk of adverse effects. Appropriate dose adjustments or replacement with a suitable alternative may be required based on the clinical condition.
    Hyperlipidemia: This medicine should be used with caution in patients with hyperlipidemia due to the increased risk of an elevation of serum triglyceride and cholesterol levels. Appropriate dose adjustments or replacement with a suitable alternative may be necessary based on the clinical condition of the patient.

  3. Interaction with Foods: Coadministration of multivitamin with minerals and this medicine should be avoided since it may alter the desired effect of Metpure-XL 12.5 Tablet. A time gap of at least 2 hours should be maintained between the administration of multivitamin with minerals and this medicine to avoid their interaction.

 

Contraindications:

  • Cardiogenic shock
  • Severe heart diseases
  • Anuria (absence of urination)
  • Hypersensitivity

Warnings: 

  • Ischemic heart disease: This medicine should not be discontinued abruptly in patients suffering from ischemic heart disease since abrupt discontinuation may worsen the patient's condition. The dosage should be reduced gradually over a period of 1 to 2 weeks along with close monitoring of heart function before the discontinuation of therapy.
  • Major surgery: This medicine should be used with extreme caution in patients undergoing a major surgery due to the increased risk of severe adverse effects. Close monitoring of heart function is recommended for such patients.
  • Pheochromocytoma: This medicine should be administered with extreme caution in patients suffering from pheochromocytoma, a condition which results in an excess of a substance that controls heartbeat and blood pressure. Appropriate corrective measures, dose adjustments, or replacement with a suitable alternative may be necessary based on the clinical condition of the patient.
  • Bronchospastic disease: This medicine should be administered with caution in patients with a disease of the lung or the airways leading to the lungs such as pneumonia, asthma, or chronic obstructive pulmonary disease. Close monitoring of lung function is recommended for such patients. Appropriate dose adjustments or replacement with a suitable alternative may be required in some cases based on the clinical condition.
  • Hyperthyroidism: This medicine should be administered with caution in patients suffering from hyperthyroidism since it may worsen the patient's condition. You may require a dose adjustment and more frequent monitoring of thyroid function to safely use this medicine. Replacement with a suitable alternative may be required in some cases based on the clinical condition.
  • Sports: Athletes are advised not to indulge in certain sports while using this medicine. Archery, racing, shooting and other high-risk sports should be avoided. Sportspersons should refer to the list of medicines published by World Anti-Doping Agency (WADA) which are not to be taken while participating in certain specific sports events.
  • Driving or operating machinery: Use of this medicine may cause dizziness, tiredness, blurring of vision, etc. in some patients. Hence, it is advised that you do not perform any activities that require high mental alertness such as driving a vehicle or operating heavy machinery if you experience any of these symptoms.
  • Pregnancy: This medicine is not recommended for use in pregnant women unless absolutely necessary. All the risks and benefits should be discussed with the doctor before taking this medicine.
  • Lactation: This medicine is not recommended for use in breastfeeding women unless necessary. All the risks and benefits should be discussed with the doctor before taking this medicine.
  • Safety with alcohol: Consumption of alcohol is not recommended during treatment with this medicine due to the increased risk of side effects such as dizziness, lightheadedness, and fainting. It is advised that you do not perform any activities that require high mental alertness such as driving a vehicle or operating machinery if you consume alcohol during treatment with this medicine.

General Instructions:

  • Take this medicine exactly as instructed by the doctor.
  • Do not take in larger amounts than advised/prescribed.
  • Consult the doctor if you experience any undesirable effects.
  • Do not stop taking the medicine before completion of the course/without consulting your doctor.

Storage conditions:

  • Store between 15°C-30°C.
  • Do not freeze.
  • Protect from sunlight and moisture.
